Pneumonic Plague Suspected in Russia: What to Know
A recent death involving a laboratory worker in Siberia has brought a rare and serious infectious disease back into the news.
The worker, who was employed at the Irkutsk Anti-Plague Research Institute in Russia, died after being hospitalized with severe pneumonia. Because of her work at a facility that studies plague and other infectious diseases, pneumonic plague was raised as a possible explanation for her illness.
The exact cause of death has not been confirmed.
The World Health Organization (WHO) said it first received reports of the suspected case on October 2 and has requested additional information from Russian authorities, including laboratory testing results and information about people who may have been exposed. Russian authorities have reported that there has been no recently confirmed case of plague in the Irkutsk region. Approximately 200 contacts have reportedly been placed under quarantine.
The European Centre for Disease Prevention and Control (ECDC) is also monitoring the situation. As of October 6, the agency reported no secondary cases and no evidence of sustained human-to-human transmission.

What Is Pneumonic Plague?
Plague is an infectious disease caused by the bacterium Yersinia pestis. While plague is often associated with historic pandemics, the disease still occurs naturally today.
There are several forms of plague, including bubonic, septicemic and pneumonic plague.
Pneumonic plague affects the lungs and is the most serious form of the disease. It can occur when plague bacteria spread to the lungs from another form of plague, or when someone inhales infectious respiratory droplets from an infected person or animal.
Unlike most forms of plague, pneumonic plague can spread from person to person. Transmission generally occurs through close contact with an infected person who is coughing and producing respiratory droplets.
What Are the Symptoms?
Pneumonic plague can develop quickly. Common symptoms can include fever, headache, weakness, cough, shortness of breath and chest pain. In some cases, a person may also produce bloody or watery mucus.
Because the illness can progress rapidly, prompt medical attention is important when pneumonic plague is suspected. The disease can be treated with antibiotics, and early treatment significantly improves the chances of recovery.

How Common Is Plague Today?
Despite its place in history, plague has never completely disappeared.
Cases still occur in some parts of the world where Yersinia pestis naturally circulates among wild animals and fleas. In the United States, human cases are rare but occur primarily in rural areas of the western states.
Modern surveillance, antibiotics and public-health measures have made plague very different from the disease that caused historic pandemics.
What Does This Mean Right Now?
The situation in Russia remains under investigation, and health authorities are continuing to evaluate the available information.
At this time, there is no evidence of sustained person-to-person transmission or a wider outbreak connected to the reported death. Health agencies are continuing to monitor the situation and gather additional information.
For most people, there is no reason to change their day-to-day activities because of these reports. As with any developing health story, the most useful approach is to follow updates from reliable public-health sources and distinguish confirmed information from speculation.
